UIL Pre-Participation Physicals

Prior to participating in marching band or athletics, students must have a physical on file. Physicals are good for one year. 

Upcoming Clinic

Each spring, Castleberry ISD hosts a physical clinic at a central location within the district to ensure students are ready to participate in extra-curriculars for the following school year. Event details are below: 

  • Date: Monday, April 22

  • Time: Event starts at 3 PM. Please arrive no later than 5:30 PM to check-in. 

  • Location: Castleberry High School [Cafeteria] // 315 Churchill Road 

  • Cost: $15 (cash only) 

  • Clothing: Athletes should wear shorts, t-shirts, and tennis shoes.

All forms should be completed by a parent or guardian prior to the clinic. This includes a printed Medical History form and the RankOne online forms. Documents are linked below.
