UIL Pre-Participation Physicals

Prior to participating in athletics, marching band, or drill team, students must have a physical on file. Physicals are good for one year. 

Upcoming Clinic

Each spring, Castleberry ISD hosts a physical clinic at a central location to ensure students are ready to participate in athletics/marching band for the following school year. Event details are below: 

  • Date: Wednesday, April 30, 2024

  • Time: Event starts at 3 and runs until 6:30 PM. Please arrive no later than 6 PM to check-in. 

  • Location: Castleberry High School [Cafeteria] // 215 Churchill Road 

  • Cost: $20 (cash or card) 

  • Clothing: Students should wear a t-shirt, shorts, and bring their eyeglasses if they have a prescription. 

Printable Documents

All forms should be completed by a parent or guardian prior to the scheduled clinic. This includes a printed Medical History form and the RankOne online forms. Students may pick up a packet from their coach, director, or in the field house from one of the athletic trainers. Documents may be printed using the links below.
