Immunization Information

Overview

In accordance with board policy FFAB (LEGAL), each student attending a public school in Texas, including Castleberry ISD, shall be fully immunized against diphtheria, rubeola (measles), rubella, mumps, tetanus, and poliomyelitis.

The Texas Department of State Health Services (TDSHS) may modify or delete any of these immunizations or may require immunizations against additional diseases as a requirement for admission to any elementary or secondary school [Education Code 38.001(a), (b)].

Grades K-12

Students are required to have the following vaccines according to the immunization schedules set forth in TDSHS regulations: pertussis, hepatitis B, hepatitis A (as applicable to the grade levels specified in state rule), and varicella (chickenpox).

Grades 6-12

TDSHS requires students to have one dose of meningococcal vaccine on or after the student’s 11th birthday [25 TAC 97.63].

Passport Health/Staying Healthy Medical Services will conduct an immunization clinic during the Back to School Bash to help get Castleberry ISD students up to date for the upcoming school year.

Vaccines Available

Chicken Pox, DtaP, Hepatitis A, Hepatitis B, HIB, Meningitis (7th grade & college required), Meningitis B (recommended for college), MMR, Polio, Pneumonia and TDaP

Cost

  • Medicaid/CHIP: No charge for vaccines.

    • Must provide a paper photo copy of card.

  • Uninsured: $10 per vaccine ($40 maximum per child.)

  • Insurance Plans: Aetna, Blue Cross Blue Shield, Cigna, Humana & United Health Care PPO plans will be accepted.

    • A paper photo copy of insurance card and driver’s license must be provided to receive vaccines at no charge.

    • Electronic copies of insurance cards cannot be accepted.

  • Not Accepted: Aetna & Blue Cross Gold, Silver, or Bronze plans or HMO plans with any insurance company.

Forms of Payment

Cash or Zelle only. Bills over $20 are not accepted.

Documents

A printed paper copy of the student's current vaccine record or note from the school stating missing vaccines is required. Electronic proof will not be accepted.

Additional Information

A parent or guardian must be present. Only students attending Castleberry Independent School District, between ages 3-18, can be seen.

This clinic is being offered as a community service event and should not replace any well child checkups at the pediatrician

Older children who resist shots and children with special needs cannot be seen given the nature of this clinic. Please refer to your pediatrician.

Due to limited staff and supplies, the clinic may end early if resources run out. Passport Health cannot guarantee that all students will be seen.
