Employee Complaint and Grievance Process

Refer to board policy DGBA(LOCAL) for detailed information about the complaint and grievance process.

Employees may use the form below to initiate the formal complaint process. 

Employee Complaint Form

If the employee does not receive the requested relief at Level One, or if the time for a decision has expired, the employee may request a hearing at Level Two to appeal the Level One decision.

Employee Level Two Appeal Form

If the employee did not receive the relief requested at Level Two or if the time for a decision has expired, the employee may appeal the decision to the Board.

Employee Level Three Appeal Form
