Immunization Schedule
The immunization schedule is listed below.
Diphtheria – Students entering Kindergarten- five doses, unless
fourth dose was administered on or after the 4th birthday.
Tetanus (Td)- One dose within the last 10 years of the last DTP/DTaP
dose.
Polio – Students entering Kindergarten- four doses, unless third dose was administered on or after the 4th
birthday. (Not required for persons 18 years or older)
Measles, Rubella and Mumps (MMR)- Two doses by age 5 or
Kindergarten entry. Physician confirmation will no longer be acceptable in
lieu of vaccinations. Only laboratory confirmation will be acceptable in
lieu of vaccinations.
Haemophilus Influenzae (HIB) – Required for students age 4 and
under. Series required with booster at age 12 months or 1 dose if given
after 15 months.
Hepatitis B – Series of three doses for students attending K-12
grades.
Varicella – One dose for students attending K-12 grades. Two doses if
vaccine given at 13 years or older. Laboratory proof or documentation of
previous illness may substitute for vaccination.
Hepatitus A- One - two doses required for entrance to PreKindergarten.
Pneumococcal (PCV7)- Required for entrance to PreKindergraten.
